If you need to change your business address (sidlo) at the Živno office, here’s how you do it.
The Živno clerks do not speak English, so you can download the Czech/English translated instructions below to show them exactly what you want.
The translated instructions (unless your Czech is good enough to communicate with the Živno office ladies, then you don’t need it).
the Business Address form signed by your new landlord or a sidlo company, giving permission to use their address as your business address
your original Živno or a copy
your passport or residency card

Go to the same office where your Živno was initially registered—(this is not necessarily where you live, but the office where you or your representative originally applied. It will say on your Živno, click here for example). The offices are open without appointment Mon & Wed 8:00-12:00, 13:00-17:30. The earlier you go the shorter the wait will be.
At the touch screen, request Živnostensky odbor >> Registrace. When your number is called, go to the appropriate office. Give them all the documents above, with the translated instructions on top, so they won’t start firing Czech at you.
They should write a date on your translated instructions when you can return to pick up your new Živno (with your new address on it). It’s usually about a week.
Return to pick up your Živno one week later.
